GENERAL DIRECTIONS

1. Please use ink and print neatly or type.
2. Complete Section A through C. Complete Section D ONLY, if the Dam was leased at the time of breeding and/or kittening.
3. A signed lease agreement must be sent to (or already on file with) the Head Registrar, prior to the litter Registration application.
4. The Dam's owner's Cattery name (or the lessee's) will appear as a prefix on the kitten's Registered name.
5. All signatures (where required) MUST be signed or the application will be returned.
6. Completed individual kitten registrations will be mailed to the litter owner (Owner of Dam or Lessee).
7. Section A (Birth information must be completed in full. Any incomplete applications will be returned. Mail all to TCA, Inc. Head Registrar, PO Box 178, Heisson, WA 98622-0178.
